Send a Case

To send a case, please follow the steps below.
 
1) If you haven't already, please fill out the New Account Form and submit it to our team. You can scan and/or email it to jennifer@hermitagelab.com, or fax it to (615) 889-4953.
 
2) After you've sent over your New Account Form, fill out the Rx Form for the necessary prescription.
